Can personal health information be used or disclosed without a patient's consent under the Privacy Rule?

Without patient consent, the Privacy Rule permits the use or disclosure of health information in limited situations such as for the individual's treatment, payment, healthcare operations, to persons involved in the patient's care, for specific public health efforts, and some research scenarios.
